Your input matters. This short, anonymous survey asks about your experience using test strips and the results you’ve seen. This information helps local harm reduction programs better serve our communities.

At the end of the survey, you can request free fentanyl, xylazine, and medetomidine test strips. Survey responses are anonymous, but some personally identifying information may be needed to fulfill mail requests.
